Procedure To Begin Services

Client intake procedures to begin services with Island Temporary Nursing are listed below:

Initial Inquiry

Please feel free to call our office at 808-791-5825 from Monday - Friday (8 am - 5 pm) with any inquiries you have. If you are interested in learning more about our in-home care services, we will send you a pamphlet (available in both English and Japanese) which includes more information about our rates and services.

Service Forms

If you are interested in beginning services with ITN, we will send the required service forms via mail, email, or hand delivery. Please return the forms upon completing them.

Medical Records

Along with the service forms, ITN requires medical records from your physician. Please complete the Hawaii HIPAA Authorization For Release Of Information Sheet (included in the service forms we send) and provide it to your physician when requesting the client's medical records.

Assessment by Registered Nurse

Once we have received ther service forms and medical records necessary, we will set up an intitial evaluation with a Registered Nurse. The nurse will visit the client's home to conduct an assessment to determine the level of care the client requires.

Services May Begin

Upon reviewing the intial assessment, Island Temporary Nursing will determine the level of care and types of service the client needs. Once we have determined that services can commence, ITN will match the client with caregivers that can work under the desired schedule and provide the necessary care.
